FREE Immunization Shots

Please bring shot record,  if you do not have it- please call to reserve an appointment, so they can access your iCare registry online for free and verify your immunization shot record,  call at 312-746-9096. leave a message if no answer.
Appointments are preferred,  15 minute increments to allow for proper cleaning between visits.
Free to all, insurance not needed,  open to all Cook County residents.
